Community Linkage Specialist

The Community Linkage Specialist position is focused on improving health outcomes of recently released HIV positive individuals from New York State Prisons through continuous engagement with medical care and support services from 30-days up to 6 months after discharge after being released in coordination with the Department of Corrections and Community Supervision (DoCCS) medical / Discharge Planning Unit (DPU) to address reentry needs and benefits navigation. Click here for full position description.

Care Coordinators

Care Coordinators will organize care and provide guidance to clients with complex psychiatric and/or co-morbid medical conditions;  develop effective interpersonal relationships with clients and work collaboratively with the interdisciplinary care team to improve health outcomes; and utilize internal and community resources to educate clients and form a care plan with specific health outcomes. Click here for full position description.

Salary: $40,000/year
